A government form for the circulation of essential workers now available

With the curfew in place, many essential workers will need to have proof of travel with them between 8 p.m. and 5 a.m. The Government of Quebec has developed a form for this purpose, available here.

The form is called ” Formulaire d’attestation de déplacements durant le couvre-feu “. The form asks the employer to certify that, during the curfew period decreed by the Government of Quebec, the person identified in the document is recognized by the organization as being an essential resource for the achievement of its commitments or activities.
